Position Overview

Qualifications

To be considered for this position, candidates must meet the following criteria:

  • Possess a **Master's Degree** in Social Work from an accredited institution.
  • Hold a valid **California Clinical Social Worker License** upon hiring.
  • Have a minimum of **2 years** of experience in a healthcare-related environment, such as Acute, Ambulatory, or Post-Acute care.
  • Demonstrate at least **2 years** of experience in **Acute Care Management** or successful completion of a recognized **Transitions in Practice (TIP)** program for Care Managers.
  • Exhibit strong expertise in **discharge planning** processes.
  • Maintain current experience in **Inpatient Social Work** settings.
