- Demonstrate at least **2 years** of experience in **Acute Care Management** or successful completion of a recognized **Transitions in Practice (TIP)** program for Care Managers.
- Exhibit strong expertise in **discharge planning** processes.
- Maintain current experience in **Inpatient Social Work** settings.
