Key Responsibilities

The Case Manager is responsible for:

  • Conducting targeted outreach and educational initiatives aimed at Latino communities to enhance awareness of mental health and wellness.
  • Executing routine case management tasks in conjunction with other program personnel to deliver comprehensive services to clients.
Essential Duties

The essential functions of this role include:

  • Assisting clients in accessing necessary community resources and support systems.
  • Facilitating the establishment of services as outlined by organizational and funding guidelines.
  • Collaborating with team members to coordinate activities, recruit participants, and manage event logistics.
  • Leading therapeutic group sessions and working alongside colleagues to develop and implement effective curricula.
  • Providing individual rehabilitation sessions focused on coping strategies to promote mental well-being.
  • Participating in regular team meetings to discuss progress and strategies.
  • Documenting all advocacy and case management activities as mandated by organizational policies and funding requirements.
Qualifications

Knowledge, Skills & Abilities:

  • Fluency in both Spanish and English is essential.
  • A strong interest in immigration matters and family dynamics is advantageous.
  • Self-motivated with the ability to engage effectively with diverse populations.
  • Exceptional organizational and administrative skills are required.

Experience:

  • Previous experience in providing mental health and substance use case management services, particularly within at-risk communities, is preferred.

Education:

  • A Bachelor's degree in social work, human services, or a related field from an accredited institution is required.

Additional Information

This position does not have supervisory responsibilities. Proficiency in MS Office Suite and a valid California Class C Driver License are required. The role may involve travel to meet clients and transport them as necessary.
