Schedule: 40 hours per week

Job Overview:

Horizon House, a prominent non-profit organization, is renowned for its commitment to providing community-oriented behavioral health, intellectual disability, and homelessness services throughout Eastern Pennsylvania and Delaware.

Each year, Horizon House serves over 4,500 adults facing psychiatric or developmental challenges, substance use disorders, and homelessness across more than 100 locations.

As a leader in community-based rehabilitation, Horizon House is celebrated for its innovative service delivery model that prioritizes community integration.

Currently, we are looking for a full-time Community Engagement Specialist to join our Developmental Services Division.

Key Responsibilities:
  • Assist individuals in the community to access essential services that promote their ability to live, learn, work, and socialize effectively.
  • Provide support to clients, ensuring continuity of care under the direct supervision of the Program Director.
  • Perform additional duties as assigned.

Qualifications:
  • A.A. degree in social sciences or a healthcare-related field, along with one year of relevant experience in the mental health or developmental disabilities sector; or a high school diploma with two years of experience in the mental health field; or a combination of education and experience that demonstrates the necessary knowledge, skills, and abilities.
  • Excellent communication skills.
  • Ability to work both independently and collaboratively within a focused team.
  • Physical capability to lift, carry, kneel, stack, stoop, and bend, which may involve direct interaction with clients.
  • Possession of manual dexterity and fine motor skills.
  • A valid driver's license is required.
  • Ability to implement services as outlined in individual program plans.
  • Participate in identifying service delivery gaps and propose actionable solutions under the guidance of the Team Coordinator.
  • Maintain accurate program reports and documentation, including medical logs and incident reports.
  • Coordinate leisure and recreational activities for clients.
  • Be prepared for frequent crisis intervention in emergency situations.
  • May be required to report to work during emergencies, including adverse weather conditions.
