Essential Functions:

- Assess the needs of individuals and families to identify necessary resources.

- Connect clients with suitable community resources and facilitate referrals.

- Advocate for clients within social service systems.

- Organize community programs and workshops to educate on available services.

- Collaborate with social workers and healthcare professionals to address community challenges.

- Maintain accurate documentation of client cases, including services rendered and progress.

- Provide crisis intervention and immediate support when necessary.

- Develop programs targeting social issues such as homelessness and substance abuse.

- Monitor client progress and adjust treatment plans as required.

- Support administrative staff and volunteers in their roles.

- Attend community networking events to foster relationships.

- Conduct home visits to engage with program participants.

Clinical Responsibilities:

- Stay informed about relevant laws and regulations affecting service delivery.

- Enhance professional skills through ongoing education and supervision.

- Refer clients to additional community resources as necessary.

Qualifications:

- A Bachelor’s degree in social work, psychology, or a related field; a Master’s degree is preferred.

- At least one year of experience in community work with non-profit organizations.

- Experience in counseling individuals or families dealing with mental health or substance use issues.

- Valid background checks and clearances are required.

- A valid driver’s license is necessary.
