Job Overview

Position Title: Housing Support Specialist FLSA Status: Non-Exempt

We are excited to announce an opening for a Housing Support Specialist.

The Housing Support Specialist will collaborate within a team framework to deliver comprehensive support services to adults with significant mental health issues, aiding them in achieving independent living and engaging with the broader community.

WORK SCHEDULE

This position has the following schedule:

  • Thursday-Saturday, 7:00 AM - 5:00 PM

KEY RESPONSIBILITIES

  • Assist clients in accordance with their individualized treatment plans, including support with hygiene, meal preparation, daily skills training, and community activities.
  • Conduct weekly assessments of living spaces for cleanliness.
  • Coordinate client needs, including appointments and engagement with community support systems and vocational services.
  • Provide immediate crisis intervention and resolution as required.
  • Review daily logs and documentation after each shift, communicating any emergencies to other staff members.
  • Supervise medication distribution visually as necessary.
  • Lead designated life skills groups.
  • Maintain client records in compliance with agency, county, state, and federal regulations.
  • Provide feedback to the primary psychiatric provider and/or case manager regarding client progress and treatment plans.
  • Facilitate assigned group activities and special client events such as outings and celebrations.
  • Assist with admission and discharge planning for community-based supported housing programs as directed.
  • Coordinate client orientation and inform property management of maintenance needs.
  • Attend scheduled staff and community meetings as assigned.
  • Commit to a minimum of 10 hours of annual continuing education/training.
  • Uphold client and staff confidentiality at all times.

BASIC QUALIFICATIONS

  • High School Diploma/GED with at least 2 years of relevant experience.
  • Current First Aid and CPR certification.
  • Current Food Handler permit.
  • Legible handwriting.
  • Basic mathematical skills.
  • Ability to read and understand agency/client documentation.
  • Valid driver's license with a clean driving record for agency vehicle use.
  • Experience with the target population or willingness to undergo specialized training.
  • Ability to work both independently and collaboratively.
  • Substance-free.
  • Capability to perform the duties outlined in this description with reasonable accommodations.

PREFERRED QUALIFICATIONS

  • Bachelor's degree in social work or a related field.
  • Familiarity with and adherence to all agency policies and procedures.
