Job Overview

POSITION SUMMARY

The Housing Case Manager (HCM) plays a vital role in assisting residents of Plymouth by being an integral member of a dedicated onsite permanent supportive housing (PSH) team. This team is focused on enhancing the quality of life for residents while ensuring the delivery of high-quality services that align with Plymouth's mission. The HCM embodies the principles of cultural proficiency, harm reduction, trauma-informed care, and the 'housing first' approach. Key responsibilities include identifying barriers to housing stability, collaborating with residents to develop Housing Service Plans, and providing care coordination and supportive services to help residents achieve their goals. This role also emphasizes the importance of community building and maintaining a consistent staff presence within the housing facilities.

Work Schedule: The HCM may work up to a 40-hour week, which may include weekday evenings and one weekend day, as well as select holiday mornings.

KEY RESPONSIBILITIES

Supportive Services

  • Collaborate with residents to create and maintain a Housing Service Plan that addresses their transition and long-term housing stability needs.
  • Possess a comprehensive understanding of available resources within Plymouth and the wider community, ensuring residents can access these resources effectively.
  • Assist residents in scheduling and attending appointments, including providing transportation when necessary.
  • Advocate for residents by consulting and collaborating with community service providers to ensure seamless care.
  • Support residents during their move-in and move-out processes in accordance with established policies.
  • Utilize de-escalation techniques to mediate conflicts and manage crises effectively.
  • Respond to emergencies and intervene as necessary, focusing on harm reduction and trauma-informed practices.
  • Address resident complaints and concerns, escalating them to the Residential Services Manager as needed.
  • Coordinate with the internal behavioral health team to ensure cohesive service delivery.

Community Engagement and Collaboration

  • Participate in scheduled supervision and team meetings to discuss resident-related issues and program objectives.
  • Foster a cooperative work environment that promotes teamwork among colleagues and community partners, including participation in community events.
  • Engage with other departments within Plymouth Housing and contribute to committees or workgroups as appropriate.
  • Provide support to other Social Services staff in different buildings as required.
  • Ensure a safe and sanitary environment for both residents and staff.

Administrative Duties

  • Adhere to the standards and requirements set forth in service program agreements, focusing on goals related to residential stability and eviction prevention.
  • Maintain accurate and timely records in databases, including daily activities and resident progress notes, while ensuring confidentiality.
  • Document significant events and building operations accurately in daily logs and assist with other program documentation as needed.
  • Manage financial documentation related to resident purchases as required.
  • Follow agency policies and procedures, providing input for program improvements as necessary.
  • Coordinate with internal departments regarding activities and resident information collection.

ADDITIONAL RESPONSIBILITIES

Perform other duties as assigned by the Residential Services Manager or Site Director.

QUALIFICATIONS

Education: A BA/BS degree in a human services field is required. Advanced education may substitute for experience, and relevant experience may also substitute for the degree requirement.

Experience: A minimum of two years of experience working with homeless and low-income populations, including individuals with mental and physical health challenges, substance use disorders, and disabilities.

Licensure/Certification: A valid Washington State Driver License and an insurable driving record are required.

  • Excellent organizational and time management abilities, with a capacity to prioritize tasks effectively.
  • Self-motivated with a knack for creative problem-solving.
  • Ability to build and maintain positive relationships with residents and community organizations.
  • Empathy and patience when working with individuals facing significant challenges.
  • Capability to work effectively under pressure with diverse populations.
  • Flexibility and a collaborative spirit when working with team members.
  • Proficiency in Microsoft Office applications and experience with databases and online research.
  • Attention to detail is crucial.

The ability and willingness to transport residents to appointments as needed is essential.

DESIRED QUALIFICATIONS

  • Experience in a Permanent Supportive Housing setting is preferred.
