Position Overview

The Residential Support Coordinator plays a vital role in assisting residents at Plymouth Housing by being an integral part of a dedicated onsite team focused on permanent supportive housing (PSH). This role is centered around enhancing the quality of life for residents while ensuring that service delivery aligns with Plymouth's mission, emphasizing cultural competence, harm reduction strategies, and trauma-informed practices.

Key Responsibilities

  • Collaborate with residents to formulate and maintain a comprehensive Housing Service Plan that addresses both immediate and long-term housing stability goals.
  • Possess a thorough understanding of available resources within Plymouth and the broader community, facilitating residents' access to essential services.
  • Assist residents in coordinating appointments and provide transportation as necessary, ensuring they receive the support they need.
  • Act as an advocate and liaison with community service providers to promote continuity of care.
  • Support residents during transitions in and out of housing, adhering to established policies and procedures.
  • Utilize de-escalation techniques to mediate conflicts and respond effectively to crises, prioritizing harm reduction and trauma-informed care.
  • Address resident concerns and complaints, escalating issues to the Residential Services Manager when appropriate.
  • Work collaboratively with the internal behavioral health team to deliver cohesive support services.

Community Engagement

  • Participate actively in team meetings and supervision sessions to discuss resident needs and program objectives.
  • Foster a cooperative work environment, promoting teamwork among colleagues and community partners.
  • Engage with other departments within Plymouth Housing and contribute to committees or workgroups as needed.
  • Provide assistance to other Social Services staff and cover front desk duties as required.
  • Ensure a safe and clean environment for both residents and staff.

Administrative Duties

  • Adhere to service program agreements, focusing on goals related to residential stability and eviction prevention.
  • Maintain accurate and timely records of daily activities, progress notes, and changes in resident status, ensuring confidentiality.
  • Document significant events and operational activities in daily logs and assist with program documentation as necessary.
  • Manage financial documentation related to resident purchases as required.
  • Comply with agency policies and procedures, providing input for improvements as needed.

Qualifications

Education: A Bachelor's degree in a human services field is required. Advanced education may substitute for experience, and relevant experience may substitute for the degree.

Experience: A minimum of two years of experience working with homeless and low-income populations, including individuals facing mental health challenges, substance use disorders, and disabilities.

Skills:

  • Exceptional written and verbal communication abilities.
  • Strong organizational skills with the capacity to prioritize tasks effectively.
  • Self-driven with a knack for creative problem-solving.
  • Ability to build and maintain positive relationships with residents and community resources.
  • Empathetic approach to individuals affected by trauma and addiction.
  • Proficient in Microsoft Office Suite and comfortable with various databases and technology.
