Position Overview

This role is part of a unionized workforce represented by Local 8 of the Office and Professional Employees International Union (OPEIU). The starting compensation for this position is $35.39 per hour.

Job Purpose

The Supportive Housing Case Coordinator (SHCC) plays a crucial role in assisting residents at Plymouth by being an integral member of a dedicated onsite permanent supportive housing (PSH) team. This team is focused on enhancing the well-being of residents while ensuring high-quality service delivery that aligns with Plymouth's mission, emphasizing cultural proficiency, harm reduction, trauma-informed care, and the 'housing first' approach.

Key Responsibilities

  • Collaborate with residents to develop and maintain a comprehensive Housing Service Plan that addresses their transition and long-term housing stability needs.
  • Possess an extensive understanding of available resources within Plymouth and the broader community, ensuring residents can access these resources effectively.
  • Assist residents in scheduling and attending appointments, including providing transportation when necessary.
  • Advocate for residents by consulting and collaborating with community service providers to ensure seamless continuity of care.
  • Support residents during their move-in and move-out processes, adhering to established policies and procedures.
  • Utilize de-escalation techniques to mediate conflicts and intervene in crises, ensuring a focus on harm reduction and trauma-informed care.
  • Address resident complaints and concerns, escalating issues to the Residential Services Manager as required.
  • Coordinate with the internal behavioral health team to provide cohesive support services to tenants.

Team Collaboration and Community Engagement

  • Participate in scheduled supervision and team meetings to discuss resident-related issues and program objectives.
  • Foster a cooperative work environment and promote teamwork among colleagues and community partners.
  • Engage with other Plymouth Housing departments and contribute to committees or workgroups as needed.
  • Provide backup support to other Social Services staff in different buildings and assist with front desk coverage as necessary.
  • Ensure a safe and sanitary environment for both tenants and staff within the buildings.

Administrative Duties

  • Adhere to service program agreements, focusing on goals related to residential stability, eviction prevention, and enhanced self-determination.
  • Maintain accurate and timely records of all activities, including progress notes and changes in resident status, while upholding strict confidentiality.
  • Document significant events and operational activities accurately in daily logs and ensure all necessary information is communicated effectively.
  • Manage financial documentation related to resident purchases as required.
  • Follow agency policies and procedures, providing input on program improvements as needed.

Qualifications

Education: A Bachelor’s degree in a human services field is required. Advanced education may substitute for experience, and relevant experience may substitute for the degree.

Experience: A minimum of two years of experience working with homeless and low-income populations, including individuals with mental and physical health challenges, substance use disorders, and disabilities.

Licensure: A valid Washington State Driver's License and an insurable driving record are required.

  • Demonstrated leadership skills that contribute to achieving team goals.
  • Proficiency in Microsoft Office applications and experience with customized databases.
  • Attention to detail and a commitment to maintaining high standards of work.
  • Willingness to transport residents to appointments as needed.

Preferred Qualifications

  • Experience in a Permanent Supportive Housing setting is highly desirable.
