Position Overview

Job Title: Residential Support Coordinator FLSA: Non-Exempt

Compensation: $25.00-$30.39 per hour

Supervisor: ISH/SSH Supervisor

We are excited to announce an opening for a Residential Support Coordinator.

The Residential Support Coordinator will collaborate with a multidisciplinary team to deliver comprehensive support services to adults with significant mental health needs, empowering them to live independently and engage with their community.

WORK SCHEDULE

Two shifts are available:

  1. Sunday-Thursday, totaling 35 hours per week. The standard shift runs from 11:00 AM to 6:00 PM, with Sunday hours from 11:00 AM to 4:00 PM and Wednesday hours from 9:00 AM to 6:00 PM. An alternative schedule includes Wednesday from 9:00 AM to 4:00 PM and Sunday from 11:00 AM to 6:00 PM.

KEY RESPONSIBILITIES

  • Assist clients in accordance with personalized treatment plans, including support with hygiene, meal preparation, daily skills training, and community engagement activities.
  • Conduct weekly assessments of living environments for cleanliness and safety.
  • Coordinate client needs, including scheduling appointments and connecting with community resources and vocational services.
  • Provide immediate crisis intervention and support as required.
  • Review daily logs and documentation post-shift, ensuring effective communication with team members regarding any urgent matters.
  • Supervise medication distribution as necessary.
  • Facilitate assigned life skills groups.
  • Maintain accurate client records in compliance with agency, county, state, and federal regulations.
  • Offer insights to primary psychiatric providers and/or case managers regarding client progress and treatment plans.
  • Organize and lead group activities and special events for clients, such as outings and celebrations.
  • Assist with admission and discharge planning for community-based housing programs as directed.
  • Coordinate client orientation and communicate with property management regarding maintenance needs.
  • Participate in scheduled staff and community meetings as assigned.
  • Commit to a minimum of 10 hours of annual professional development and training.
  • Uphold confidentiality for both clients and staff at all times.

BASIC QUALIFICATIONS

  • High School Diploma or GED with at least 2 years of relevant experience.
  • Current First Aid and CPR certification.
  • Valid Food Handler permit.
  • Legible handwriting and basic mathematical skills.
  • Ability to read and understand agency/client documentation.
  • Valid driver's license with a clean driving record for agency vehicle use.
  • Experience with the target population or willingness to undergo specialized training.
  • Ability to work both independently and collaboratively within a team.
  • Must be free from substance use.
  • Capability to perform job duties as outlined with reasonable accommodations.

PREFERRED QUALIFICATIONS

  • Bachelor's degree in social work or a related field.
  • Familiarity with agency policies and procedures.
