POSITION SUMMARY

The Housing Case Manager (HCM) plays a vital role in assisting residents of Plymouth by being an integral member of a dedicated permanent supportive housing (PSH) team. This role is focused on enhancing the well-being of residents and ensuring the delivery of high-quality services aligned with Plymouth's mission. The HCM employs principles of cultural proficiency, harm reduction, trauma-informed care, and the 'housing first' philosophy. Key responsibilities include identifying barriers to housing stability, collaborating with residents to develop Housing Service Plans, and providing care coordination and supportive services to help residents achieve their goals. This role also fosters a sense of community while ensuring staff presence in residential buildings.

Work Schedule: The position may require up to a 40-hour work week, including evening shifts and weekend availability, along with select holiday mornings.

KEY RESPONSIBILITIES

Supportive Services

  • Collaborate with residents to create and maintain a Housing Service Plan that addresses their transition and long-term housing stability needs.
  • Maintain a comprehensive understanding of available resources within Plymouth and the broader community to assist residents in accessing necessary support.
  • Assist residents with scheduling appointments, arranging transportation, and accompanying them as needed.
  • Advocate for residents by coordinating with community providers to ensure continuity of care.
  • Support residents during their move-in and move-out processes, adhering to Plymouth's policies.
  • Utilize de-escalation techniques to mediate conflicts and intervene in crises, focusing on harm reduction and trauma-informed care.
  • Address resident complaints and concerns, escalating issues to the Residential Services Manager when necessary.
  • Work with the internal behavioral health team to ensure cohesive service delivery to tenants.

Community Engagement and Collaboration

  • Participate in scheduled supervision and team meetings to discuss resident and program-related issues.
  • Promote teamwork and collaboration among colleagues and community partners, including supporting community events.
  • Engage with other Plymouth Housing Departments and participate in relevant committees or workgroups.
  • Provide support to Social Services staff in other buildings as needed.
  • Ensure a safe and sanitary environment for tenants and staff.

Administrative Duties

  • Adhere to service program agreements, focusing on residential stability, eviction prevention, and self-determination.
  • Maintain accurate and timely documentation of daily activities, progress notes, and changes in resident status while ensuring confidentiality.
  • Record significant events involving residents and building operations in daily logs, and assist with other program documentation as required.
  • Manage financial documentation related to resident purchases.
  • Follow agency policies and procedures, providing input on program policies as needed.
  • Coordinate with internal departments for activities and resident information collection.

ADDITIONAL RESPONSIBILITIES

Perform other duties as assigned by the Residential Services Manager or Site Director.

QUALIFICATIONS

Education: A BA/BS degree in a human service field is required. Advanced education may substitute for experience, and years of experience may substitute for the degree.

Experience: A minimum of two years of experience working with homeless and low-income individuals, including those with mental and physical health challenges, substance use disorders, and disabilities.

Licensure/Certification: A valid Washington State Driver License and an insurable driving record are required.

  • Proficiency in Microsoft Office applications and experience with databases.
  • Attention to detail in all aspects of work.

Willingness to transport tenants to appointments as necessary.

DESIRED QUALIFICATIONS

  • Experience in a Permanent Supportive Housing setting is preferred.
