Position Overview: A proactive and skilled clinician dedicated to managing patient populations, demonstrating a strong ability to deliver
relationship-centered care within a collaborative healthcare team. This role involves utilizing advanced critical
thinking skills to create, execute, and assess a personalized care plan that informs, empowers, and enhances
patient outcomes across all stages of care.

Key Responsibilities:

  • After-Hours Support: Provide essential after-hours and weekend coverage, acting as the primary contact for pre and post-transplant patients as well as internal and external healthcare providers.
  • Clinical Assessment: Conduct thorough telephone assessments leveraging a robust clinical knowledge base, ensuring effective care coordination and treatment based on comprehensive evaluations.
  • Patient Education: Inform and educate patients, families, and legal guardians about various aspects of the transplant process, including living donation options, evaluation procedures, organ allocation, and post-operative care.
  • Care Accessibility: Address the availability and accessibility of specialized care required for transplant patients and their families.
  • Safety and Efficacy: Consider factors related to safety, effectiveness, cost, and overall impact when planning and delivering care.
  • Expert Care Delivery: Engage in clinical assessment, planning, prioritization, coordination, and implementation of care.
  • Ethical Practice: Maintain knowledge of ethical principles in transplantation and utilize available resources to evaluate the merits and risks associated with transplant procedures.
  • Consultation and Education: Provide guidance and education to healthcare professionals and the broader community.
  • Leadership Development: Actively participate in shared governance and demonstrate leadership within the healthcare setting.
  • Professional Advancement: Promote the growth of the transplant profession through information dissemination, policy advocacy, and participation in decision-making processes.
  • Lifelong Learning: Engage in continuous professional development through formal education, certification, and participation in professional organizations.
  • Team Support: Assist in onboarding new team members and provide mentorship as needed.
  • Quality Assurance: Adhere to clinical documentation standards, engage in performance improvement initiatives, and commit to maintaining compliance with regulatory standards.
  • Outcome Evaluation: Participate in assessing patient care outcomes and support initiatives aimed at improving overall patient care quality.
  • Innovation in Care: Apply technologies that enhance patient care and actively seek to implement evidence-based practices.
  • Research Participation: Engage in research and contribute to the publication of new initiatives and treatment interventions.

Compensation: Competitive hourly rate.

Minimum Qualifications:

  • Education: Bachelor of Science in Nursing from an accredited institution.
  • Experience: Minimum of 2 years of relevant clinical experience.
  • Licensure: Valid Registered Nurse license in the Commonwealth of Virginia. Certification in Basic Life Support (BLS) from the American Heart Association is required. Certification as a Transplant Coordinator is expected within two years of employment.

Physical Requirements: The role necessitates prolonged periods of standing, frequent travel, and the ability to lift and maneuver patients. Proficient communication, auditory, and visual skills are essential, along with attention to detail and legible writing.
