Clinical Nurse Specialist in Transplant Care

As a dedicated and skilled clinician, you will focus on managing patient populations while delivering relationship-centered care within a collaborative multidisciplinary environment. This role emphasizes advanced critical thinking to create, implement, and assess personalized care plans that educate and empower patients, optimizing outcomes throughout their healthcare journey.
Key Responsibilities:
1. Provide after-hours and weekend support as the primary contact for pre and post-transplant patients, as well as internal and external healthcare providers.
2. Conduct thorough telephone assessments utilizing a strong clinical knowledge base, ensuring effective care coordination and treatment based on comprehensive evaluations.
3. Facilitate communication among all members of the transplant team, ensuring timely updates on clinical decisions and patient responses.
Patient Education:
1. Educate patients, families, and legal guardians about living donation options, evaluation processes, and organ allocation.
2. Discuss the risks and benefits of transplant procedures, including post-operative care and immunosuppressive therapy.
3. Address the unique needs of transplant patients and their families, ensuring access to specialized care.
Professional Standards:
This position aligns with the ANA Nursing Scope and Standards of Practice and the ANA Code of Ethics for Nurses. Responsibilities include:
1. Engaging in quality improvement initiatives and maintaining compliance with regulatory standards.
2. Participating in the evaluation of patient care outcomes and implementing strategies to enhance overall care quality.
3. Supporting the onboarding and mentoring of new team members, fostering a culture of continuous learning and professional development.
Qualifications:
- Education: Bachelor of Science in Nursing from an accredited institution.
- Experience: Minimum of 2 years in a relevant clinical setting.
- Licensure: Active Registered Nurse license in the Commonwealth of Virginia, with BLS certification required.
Physical Requirements:
This role involves prolonged standing, frequent travel, and the ability to lift and maneuver patients as necessary. Candidates should possess strong communication skills and attention to detail.
