Position Overview

A dedicated and skilled clinician specializing in population health management, the Clinical Transplant Nurse plays a vital role in delivering relationship-centered care within a collaborative, multidisciplinary environment. This professional employs advanced analytical skills to create, execute, and assess personalized care plans aimed at educating and empowering patients, thereby enhancing health outcomes throughout various stages of care.

Key Responsibilities
  • Provide after-hours and weekend support as the primary contact for pre and post-transplant patients, as well as internal and external healthcare providers.
  • Conduct thorough telephone assessments utilizing a robust clinical knowledge base, ensuring effective care coordination and treatment based on comprehensive evaluations.
  • Facilitate patient engagement in the transplant process, including education on living donation options, evaluation protocols, and post-operative care.
  • Address the specific needs of transplant patients and their families, ensuring access to specialized care.
  • Implement safety, efficacy, and cost considerations in the planning and delivery of care.
  • Exhibit expert clinical judgment in assessing, planning, prioritizing, and coordinating care.
  • Provide education and consultation to healthcare professionals and community members regarding transplant practices.
  • Participate in shared governance and demonstrate leadership in advancing the transplant nursing profession.
  • Engage in lifelong learning through professional development opportunities and maintain relevant certifications.
  • Support the onboarding and training of new team members.
  • Contribute to quality improvement initiatives and ensure compliance with regulatory standards.
  • Participate in research and dissemination of innovative transplant care practices.
Qualifications

Education: Bachelor of Science in Nursing from an accredited institution is required.

Experience: Minimum of two years of relevant nursing experience.

Licensure: Must hold a valid Registered Nurse license in the Commonwealth of Virginia. Certification in Basic Life Support (BLS) from the American Heart Association is mandatory. American Board of Transplant Coordinator Certification is required at the time of hire or within two years of employment.

Physical Requirements

This role necessitates the ability to stand for extended periods, frequent travel, and physical activities such as bending and lifting. Strong communication, auditory, and visual skills are essential, along with attention to detail and legible writing.
