Position Overview:
We are seeking a dedicated and skilled Registered Nurse specializing in transplant care to enhance our multidisciplinary team. This role emphasizes a commitment to patient-centered care, focusing on effective population management and fostering collaborative relationships within the healthcare environment.

Key Responsibilities:
As a vital member of our transplant team, you will:
  • Utilize advanced clinical judgment to create, implement, and assess personalized care plans that empower patients and improve outcomes throughout their healthcare journey.
  • Provide after-hours and weekend support as the primary contact for pre and post-transplant patients, ensuring seamless communication with internal and external healthcare providers.
  • Conduct thorough telephone assessments, leveraging your clinical expertise to coordinate complex care and treatment based on comprehensive evaluations.
  • Educate patients and their families on critical aspects of the transplant process, including living donation options, evaluation procedures, and post-operative care.
  • Ensure that all members of the transplant team are informed of clinical decisions and patient responses in a timely manner.
  • Participate in quality assurance and performance improvement initiatives to enhance patient care outcomes.
  • Engage in ongoing professional development and maintain specialty certification to uphold the highest standards of nursing practice.

Qualifications:
  • Bachelor of Science in Nursing from an accredited institution.
  • A minimum of two years of relevant nursing experience.
  • Current licensure as a Registered Nurse in the Commonwealth of Virginia.
  • Certification in Basic Life Support (BLS) from the American Heart Association.
  • American Board of Transplant Coordinator Certification within two years of employment.

Physical Requirements:
This position requires the ability to stand for extended periods, travel frequently, and perform tasks that involve bending and lifting. Strong communication skills and attention to detail are essential for success in this role.
