Position Overview:
The Transplant Clinical Pharmacist plays a crucial role in delivering specialized, patient-focused pharmaceutical care to individuals undergoing solid organ transplants, both in inpatient settings and outpatient clinics.

Key Responsibilities:
  • Deliver comprehensive pharmaceutical care aimed at achieving optimal therapeutic results.
  • Advocate for cost-effective medication management.
  • Foster a collaborative work environment through effective communication and teamwork.
  • Assist in the verification and dispensing of medication orders.
  • Educate healthcare professionals on pharmacotherapy practices.
  • Engage in multidisciplinary drug therapy management strategies.
  • Contribute to the development and implementation of treatment protocols to enhance patient care.

Qualifications:
The ideal candidate will possess the ability to work autonomously while demonstrating a positive demeanor and a strong commitment to teamwork and customer service. A focus on continuous improvement in patient care is essential, achieved through collaboration within the transplant pharmacy team.

Clinical Focus:
Provide clinical pharmaceutical services to patients, particularly those undergoing abdominal, heart, and lung transplants, in both inpatient and outpatient settings.

Professional Development:
Support the educational growth of the Pharmacy department and engage in ongoing professional self-improvement.

Additional Duties:
Oversee the daily operations of the transplant transitions of care technician and participate in weekend rotations to provide clinical transplant pharmacy services.

Compensation:
Competitive hourly rate ranging from $52.46 to $83.94, commensurate with experience.

Minimum Requirements:
  • Bachelor's degree in Pharmacy or Pharm.D. required.
  • Completion of a PGY-1 residency or equivalent hospital experience; PGY-2 residency preferred for specialty areas.
  • Active license from the Commonwealth of Virginia State Board of Pharmacy.
  • Current BLS certification from the American Heart Association.

Physical Demands:
This role requires prolonged periods of sitting and repetitive motions such as typing. Strong communicative, auditory, and visual skills are essential, along with attention to detail and the ability to write clearly. The position may involve lifting, pushing, or pulling as necessary.
