Job Responsibilities

As a Clinical Nurse Care Coordinator, your primary role will be to advocate for patient-centered care by orchestrating the care plan throughout the patient's stay, managing their length of stay, ensuring optimal resource utilization, and developing a safe and effective discharge plan in collaboration with the multidisciplinary team. You will facilitate the transition of care using established criteria alongside the healthcare team, promoting quality outcomes and patient throughput while balancing optimal care with resource efficiency.

  • Deliver case management services for both inpatient and observation patients as designated.
  • Identify patients at risk for adverse outcomes during transitions between care levels or settings.
  • Conduct comprehensive assessments of the psychosocial, medical, and discharge needs of patients and their families, alongside evaluating available resources.
  • Reassess patients' clinical conditions as necessary, considering readmission risks and developing strategies to mitigate them, including education on healthcare access and community resources.
  • Coordinate care plans and drive discharge planning by collaborating with the multidisciplinary healthcare team, particularly the patient's physician, to ensure a successful transition.
  • Work closely with Social Services to confirm that post-acute medical needs and care levels are appropriate.

Qualifications

  • Minimum of 2 years' experience in case management or 3 years' experience in clinical nursing.
  • Experience with InterQual is preferred.

Education

  • Associate Degree in Nursing or Nursing Diploma.
  • Bachelor's Degree in Nursing is preferred.

Licenses and Certifications

  • Active Registered Nurse license in the state(s) of practice in accordance with regulations.
  • Certification in Case Management is preferred.

Work Schedule

  • 32 hours per week, with rotating weekends (1 weekend every 3 weeks).
