Job Summary and Qualifications
As a Clinical Nurse Care Coordinator, your responsibilities will include:
  • Facilitating patient-centered care by coordinating comprehensive care plans and managing patient transitions.
  • Collaborating with multidisciplinary teams to ensure optimal resource management and safe discharge planning.
  • Conducting thorough assessments of patients' psychosocial, medical, and discharge needs.
  • Identifying patients at risk for adverse outcomes and developing strategies to mitigate these risks.
  • Partnering with Social Services to address post-acute medical needs.

Qualifications:
  • Minimum of 2 years of experience in case management or 3 years in clinical nursing.
  • Preferred experience with InterQual.
  • Associate Degree in Nursing or Nursing Diploma; Bachelor's Degree in Nursing preferred.
  • Current Registered Nurse license in the state of practice.
  • Certification in Case Management is preferred.
