Job Responsibilities and Qualifications

As a Clinical Nurse Case Manager, you will play a vital role in promoting patient-centered care by coordinating care plans, managing patient stays, ensuring efficient resource management, and developing safe discharge plans in collaboration with a multidisciplinary team. Your responsibilities will include:

  • Delivering case management services for both inpatient and observation patients as assigned.
  • Identifying patients at risk for adverse outcomes during transitions between care levels or settings.
  • Conducting comprehensive assessments of psychosocial, medical, and discharge needs for patients and their families, along with evaluating available resources.
  • Reassessing patients' clinical conditions as necessary, considering readmission risks and developing strategies to mitigate these risks through education on healthcare access and community resources.
  • Coordinating care plans and driving discharge processes by collaborating with the healthcare team, particularly the patient's physician, to ensure successful transitions.
  • Partnering with Social Services to confirm that post-acute medical needs and care levels are appropriate.
Required Qualifications
  • A minimum of 2 years of experience in case management or 3 years in clinical nursing.
  • Preferred experience with InterQual.
  • An Associate Degree in Nursing or a Nursing Diploma.
  • A Bachelor's Degree in Nursing is preferred.
  • Current licensure as a Registered Nurse in the state(s) of practice according to applicable laws and regulations.
  • Certification in Case Management is preferred.
