Position Overview

We are looking for a dedicated Patient Care Coordinator to join our team, ensuring that we maintain high standards of quality and efficient care for all patients.

As a Patient Care Coordinator, your responsibilities will include:

  • Promoting patient-centered care by coordinating care plans throughout the patient's stay.
  • Managing the duration of patient stays and ensuring optimal resource utilization.
  • Developing safe and effective discharge plans in collaboration with a multidisciplinary team.
  • Facilitating transitions of care and monitoring patients at risk for adverse outcomes.
  • Conducting comprehensive assessments of patients' psychosocial, medical, and discharge needs.
  • Identifying readmission risks and implementing strategies to mitigate them through education and resource access.
  • Collaborating with healthcare professionals to drive successful care transitions.
  • Partnering with Social Services to address post-acute medical needs.

Qualifications

To be considered for this role, candidates should possess:

  • A minimum of 2 years of experience in case management or 3 years in clinical nursing.
  • An Associate Degree in Nursing or a Nursing Diploma; a Bachelor's Degree in Nursing is preferred.
