Position Overview
We are looking for a Clinical Nurse Case Manager to ensure the delivery of high-quality, efficient care to all patients.

As a Clinical Nurse Case Manager, your responsibilities will include:

  • Promoting patient-centered care by coordinating the care plan for patient stays.
  • Managing the length of stay and ensuring effective resource management.
  • Developing safe and appropriate discharge plans in collaboration with the multidisciplinary team.
  • Facilitating care transitions using established criteria.
  • Providing case management services for both inpatient and observation patients.
  • Identifying patients at risk for adverse outcomes during transitions of care.
  • Conducting comprehensive assessments of psychosocial, medical, and discharge needs.
  • Reassessing patients' clinical conditions as necessary.
  • Developing strategies to mitigate readmission risks.
  • Collaborating with the healthcare team to drive discharge planning.
  • Partnering with Social Services to ensure appropriate post-acute care needs.

Qualifications
To be considered for this role, candidates should possess:

  • A minimum of 2 years of experience in case management or 3 years in clinical nursing.
  • Preferred experience with InterQual.
  • An Associate Degree in Nursing or a Nursing Diploma; a Bachelor's Degree in Nursing is preferred.
  • A current Registered Nurse license in the state of practice.
  • Certification in Case Management is preferred.

Schedule
This position requires 32 hours of work with rotating weekends.
