Position: Community Support Coordinator

Role Overview: The Community Support Coordinator is a vital member of an interdisciplinary team that delivers adaptable, empathetic, and community-oriented services aimed at fostering stability, recovery, and reintegration for individuals facing mental health challenges, histories of homelessness, and potential substance use issues. This role is integral to Pathways' Assertive Community Treatment (ACT) team, collaborating with a diverse staff to manage a shared client caseload and liaising with community partners.

Employment Type: Full-Time

Key Responsibilities:

  • Establish and nurture relationships with clients, employing a client-centered, harm reduction, and strengths-based approach.
  • Assist clients in accessing community resources and services, including entitlements, benefits, advocacy, and addiction support.
  • Provide individualized support and guidance on life skills, housing issues, and health management, including medication assistance and budgeting.
  • Maintain accurate written and electronic records, prepare reports, and complete necessary program documentation.
  • Participate in a 24/7 on-call service rotation with fellow team members.
  • Attend annual staff and program retreats, which are held during regular working hours and are compensated.
  • Perform additional duties as assigned.

Qualifications:

  • Strong interpersonal skills and a solid foundation in supportive counseling techniques, with the ability to connect with marginalized populations.
  • Exceptional communication, writing, and computer proficiency.
  • Independent problem-solving capabilities and a proactive approach to complex situations.
  • Experience in mental health or harm reduction settings, particularly in community service roles, is preferred.
  • A Bachelor's Degree is required; a Master's Degree is preferred.
  • A valid driver's license, reliable vehicle for work purposes, and current automobile insurance are necessary.
  • A willingness to engage in self-reflection, learning, and adopting new relational approaches.
  • Ability to tolerate risk and discomfort.

Compensation:

  • Hourly Rate: $20-$21
  • Hiring Bonus: $3000 available upon successful completion of the orientation period.
