Job Overview

Position: Forensic Assertive Community Treatment (FACT) Service Coordinator - $3000 Hiring Bonus Available*

Organization: Pathways Vermont is a nonprofit organization committed to addressing homelessness and delivering mental health support.

Role Summary: The Service Coordinator plays a vital role within the Forensic Assertive Community Treatment (FACT) team, providing essential community-based assistance and coordinating various services for individuals in the program. This role emphasizes helping clients secure employment, access benefits, and receive comprehensive support tailored to their needs.

Available Positions: 1

Work Arrangement: In-person

Work Schedule: Full-time, with flexible hours between 9 am - 7 pm (5 days a week for 7-8 hour shifts).

Supervision: Reports to the FACT Program Manager

Key Responsibilities:

  • Engage clients through a client-centered approach
  • Develop and maintain individualized service plans
  • Assist clients with housing-related needs
  • Stay updated on community resources
  • Provide referrals to local agencies
  • Maintain accurate records and complete necessary documentation
  • Offer personalized support and counseling
  • Coordinate referrals to community services
  • Educate clients on available benefits and entitlements
  • Participate in on-call service rotations
  • Attend staff meetings and program retreats
  • Perform additional duties as assigned

Qualifications:

  • Bachelor's degree preferred
  • Experience in mental health services is advantageous
  • Strong communication and computer skills
  • Able to build relationships with marginalized populations
  • Familiarity with community resources
  • Valid driver's license and reliable transportation
  • Experience in mental health or related fields is ideal
  • Willingness to learn and engage in self-reflection

Working Conditions/Physical Requirements:

  • Use of a computer and office equipment
  • Ability to navigate stairs for client visits
  • Occasional lifting and carrying of items up to 50 pounds
  • Meeting clients in various community settings
  • Frequent walking within the community
  • Exposure to outdoor environmental conditions
  • Potential exposure to elevated noise levels

Compensation:

  • Hourly wage: $20 - $22.00
  • $3000 Hiring Bonus

*Hiring bonus is provided after successful completion of the orientation period*
