Job Overview

At SPWA Services, we are seeking a highly skilled and dedicated County Support Coordinator to join our team. As a key member of our support services team, you will play a vital role in facilitating individuals with developmental disabilities and their families to locate and secure appropriate support services.

Key Responsibilities:

  • Develop and implement personalized support plans for individuals with developmental disabilities
  • Connect individuals and their families with approved Medicaid support providers
  • Monitor services to ensure effectiveness and make adjustments as needed
  • Collaborate with community resources and state-approved programs to provide comprehensive support services
  • Provide exceptional customer service and support to individuals and their families

Requirements:

  • Bachelor's Degree in a related field
  • Minimum of 1 year of experience working with the I/DD community
  • Strong proficiency in written and verbal communication
  • Skilled with 21st century technological devices
  • Personal laptop and adept at using online web interfaces
  • Excellent time management and follow-up skills
  • Organizational skills
