Position Overview

Role: The Community Support Coordinator is a vital member of the interdisciplinary team dedicated to providing flexible and compassionate services aimed at enhancing the stability and recovery of individuals facing mental health challenges and histories of homelessness.

Organization: Pathways Vermont is a progressive nonprofit organization focused on eradicating homelessness and delivering innovative mental health solutions through ongoing supportive social work.

Key Responsibilities:

  • Build and nurture relationships with clients, utilizing a client-centered and strengths-based approach.
  • Facilitate access to community resources and services, including entitlements, advocacy, and addiction support.
  • Offer individualized support and guidance on life skills, housing issues, and health maintenance, including medication management and budgeting assistance.
  • Maintain accurate records and documentation, ensuring compliance with program requirements.
  • Participate in a 24/7 on-call service rotation with team members.
  • Attend mandatory annual staff retreats and programmatic meetings, which are compensated.
  • Perform additional duties as assigned.

Qualifications:

  • Strong interpersonal skills and a solid foundation in supportive counseling techniques.
  • Exceptional communication, writing, and computer proficiency.
  • Demonstrated problem-solving abilities and a proactive approach to challenging situations.
  • Experience in mental health or harm reduction services is preferred.
  • A Bachelor's Degree is required; a Master's Degree is preferred.
  • A valid driver's license and reliable transportation are necessary.
  • A commitment to self-reflection and continuous learning in relationship-building.
  • Ability to tolerate risk and discomfort in various situations.

Compensation:

  • Hourly wage of $20-$21.
  • A hiring bonus of $3000 is available upon successful completion of the orientation period.
