Position Overview

This role is supported by a state grant, ensuring funding stability for the foreseeable future.

The Community Outreach Case Coordinator engages directly with individuals and families residing in emergency accommodations, as well as those utilizing daytime support services for community members facing homelessness. The coordinator will provide essential support to clients, assisting them in formulating a housing strategy aimed at achieving stable living conditions. Additionally, they will facilitate access to supplementary services and resources within the community to aid in maintaining housing stability once secured.

Key Responsibilities:

  • Establish and nurture professional relationships with clients, aiding them in navigating various challenges.
  • Conduct visits to client locations, including emergency shelters and associated facilities.
  • Assist clients in completing applications for housing assistance programs and subsidies.
  • Guide clients through the process of accessing benefits such as food assistance and financial support.
  • Connect clients with available resources to streamline their transition from emergency accommodations to permanent housing.
  • Develop crisis intervention plans that focus on early prevention strategies when housing stability is at risk.
  • Work collaboratively with referral organizations to determine client eligibility and verify necessary documentation.
  • Support clients in meeting their basic needs, including access to hygiene products and medical services.
  • Foster positive relationships with accommodation providers and other community partners.
  • Provide ongoing support for clients who have secured housing, including communication with landlords and access to community resources.

This is a full-time position with flexible scheduling options to accommodate the needs of clients, including the possibility of working outside standard office hours.

Qualifications:

  • A high school diploma or equivalent is required; a bachelor's degree in social work, human services, or a related field is preferred.
  • At least one year of experience in direct service roles is required; two years of experience along with relevant coursework is preferred, particularly in case management.
  • Experience working with individuals facing homelessness and crisis situations is advantageous.
  • Ability to engage with individuals experiencing mental health challenges, substance use issues, and other personal crises is essential.
  • Familiarity with local regulations and procedures related to human services is preferred.
  • Strong reliability, independent judgment, and excellent communication skills are necessary.
  • Willingness to travel within the local area is required.
