Position Overview:
The Outreach Specialist is tasked with engaging individuals and families experiencing homelessness through direct outreach efforts.

Key Responsibilities:
  • Conduct outreach initiatives to connect with homeless individuals residing in shelters, on the streets, in encampments, or other unsuitable living conditions through direct, face-to-face interactions.
  • Focus on homeless households or individuals facing Severe Mental Illness (SMI) or co-occurring Substance Use Disorder (SUD) and facilitate their connection to the appropriate homeless response teams and coordinated entry for housing solutions.
  • Collaborate with the Out-Wayne Continuum of Care (CoC) and neighboring CoCs to expedite housing placements for those eligible for the PATH program, including identifying individuals on Veteran and Chronic By-Name-Lists.
  • Perform initial housing assessments utilizing the SPDAT tool, ensuring the provision of suitable services by evaluating acuity and prioritizing needs.
  • Offer light-touch case management to qualifying individuals and families, securing necessary supportive services tailored to specific needs.
  • Services may encompass assistance with housing support, access to identification and eligibility documents, food, clothing, linkage to income supports through SOAR or other resources, connection to physical and mental health services, and the formulation of individualized service plans.
  • Possess knowledge of the Michigan Department for Health and Human Services (MDHHS) PATH Program.
  • Participate in all mandatory training sessions, including those mandated by Substance Abuse & Mental Health Services Administration (SAMHSA), Michigan Department of Health & Human Services (MDHHS), and Wayne Metro.
  • Work collaboratively with agency personnel to disseminate information regarding available services for the homeless and subpopulations, including access methods and useful contacts to ensure effective client support.
  • Maintain precise data and program records that align with WMCAA standards for statistical analysis, compliance with federal and state regulations, and preparation of program reports as required by management.
  • Exhibit a positive demeanor towards assigned tasks, clients, and colleagues, demonstrating a willingness to collaborate.
  • Engage with the Manager to establish a weekly schedule that aligns with PATH program objectives and identified areas, optimizing time utilization.
  • Actively contribute to staff meetings by being prepared, employing problem-solving skills, and providing constructive feedback to team members.
  • Assist the Supportive Housing and Homeless Services (SHHS) department in fulfilling the agency's mission as needed and directed by management.
  • Support Wayne Metro's mission of empowering individuals and communities to thrive and maintain adherence to agency policies and procedures.
  • Perform additional duties as assigned.
