About CareAdvisors: We are a healthcare technology organization dedicated to removing obstacles to care for at-risk and underserved populations. We are seeking a dedicated Social Care Partner to play a pivotal role in supporting our mission in INDIANA. This contract position (with potential for a permanent role) involves acting as a local advocate for the Indianapolis community while also serving as a mentor and trusted resource for Associate-level outreach team members.

Key Responsibilities:
  • Community Advocate:
    • Establish and nurture relationships within the Indianapolis communities, particularly with at-risk groups and local organizations.
    • Develop and execute effective outreach initiatives that respond to community needs.
    • Lead in-person outreach activities at community gatherings, connecting residents with essential resources.
    • Conduct home visits to provide tailored support and assistance as required.
    • Act as a liaison between CareAdvisors and local residents, ensuring their perspectives are represented.
  • Mentorship and Support:
    • Provide coaching, mentorship, and guidance to Associate Community Outreach Specialists.
    • Exemplify empathetic and compassionate leadership consistently.
    • Utilize your knowledge of local organizations and resources to enhance the skills and knowledge of Associate members.
    • Assist management with the onboarding and training of new team members.
    • Stay informed about local resources and information to facilitate knowledge-sharing.
    • Promote meticulous data tracking and documentation within our systems.
    • Collaborate with the Social Care Manager on outreach strategies to enhance community awareness of available resources.
Qualifications:
  • Must reside in Wayne or Marion County
    Willingness to travel 1-2 hours for visits (travel time compensated).
  • Bachelor's Degree in Human Services or a related field (or equivalent experience).
  • 4+ years of experience in a healthcare, social services, or community outreach environment.
  • Strong knowledge of case management, Medicaid, and government benefits (or 4+ years in behavioral health).
  • Bilingual candidates are preferred (Spanish is a plus).
  • Exceptional interpersonal and social skills to engage with diverse populations.
  • Strong written and verbal communication skills to effectively advocate for community members.
  • Valid driver's license, proof of insurance, and access to a vehicle.
  • Proficient in computer applications and open to learning new technologies.
