Position Overview:
The Outreach Specialist is tasked with performing street outreach to individuals and families experiencing homelessness.

Key Responsibilities:

1. Engage in street outreach and connect with homeless individuals residing in shelters, on the streets, in encampments, or other unsuitable locations through direct intervention.

2. Focus on homeless households or individuals facing Severe Mental Illness (SMI) or co-occurring Substance Use Disorder (SUD) and facilitate their connection to the appropriate homeless response teams or coordinated entry for housing solutions.

3. Collaborate with the Out-Wayne Continuum of Care (CoC) and adjacent CoCs to efficiently house individuals qualifying for the PATH program, including identifying individuals on Veteran and Chronic By-Name-Lists.

4. Conduct initial housing assessments utilizing the identified SPDAT, ensuring the provision of suitable services by evaluating acuity and prioritizing needs.

5. Provide light-touch case management to eligible individuals and families, seeking and securing necessary supportive services tailored to specific needs.

6. Services may encompass assistance with housing support, obtaining identification and eligibility documents, food, clothing, linking to income supports through SOAR or other means, facilitating access to physical and mental health treatment, and developing individualized service plans.

7. Maintain knowledge of the Michigan Department for Health and Human Services (MDHHS) PATH Program.

8. Attend all mandatory training sessions, including those required by the Substance Abuse & Mental Health Services Administration (SAMHSA), Michigan Department of Health & Human Services (MDHHS), and Wayne Metro.

9. Collaborate closely with Agency staff to share insights regarding available services for the homeless and subpopulations, methods to access these services, and contacts that may assist in ensuring clients receive comprehensive support.

10. Maintain precise data and program records compatible with those kept by WMCAA to derive statistics, meet federal and state mandates, and compile program reports as requested by management.

11. Uphold a positive attitude towards assigned tasks, clients, and colleagues, demonstrating a willingness to collaborate with others.

12. Work in partnership with the Manager to establish a weekly schedule that aligns with PATH program objectives and identified areas, while monitoring and optimizing time utilization.

13. Actively engage in all staff meetings, coming prepared with problem-solving skills and providing constructive feedback to team members.

14. Assist the Supportive Housing and Homeless Services (SHHS) department in various aspects of our mission as needed and directed by management.

15. Support Wayne Metro's mission of empowering individuals and communities to thrive and maintain a commitment to the Agency's policies and procedures.

16. Perform additional duties as assigned.
