Role Overview:
The Registered Nurse (RN) plays a crucial role within the primary care team, reporting directly to the Clinical Services Manager (CSM). This position supports the Advanced Practice Clinician (APC) and our members in coordinating essential care services. The RN collaborates closely with all team members and may assist multiple providers.

The RN operates under the guidance of the provider, with tasks delegated by the provider or the team CSM. The RN is knowledgeable about and supports the Care Program Models, including Institutional Special Needs Plans (ISNP), Institutional Special Needs Plans (IESNP), and Transitions to Skilled (TTS), working collaboratively with the team to achieve program goals.

This role involves providing support to our members both telephonically and in-person, whether in patient settings or home environments.

Key Responsibilities:
1. Establish and maintain effective communication and a trusting collaborative relationship with the Integrated Care Team (ICT), members, and primary caregivers.
2. Coordinate with various organizational units to ensure seamless benefit coordination, including utilization review staff and pharmacy.
3. Review member status and assist in achieving progress towards care goals; adjust the Individual Care Plan (ICP) as necessary.
4. Perform administrative tasks on behalf of the Primary Care Provider (PCP) and/or APC.
5. Participate in analyzing program effectiveness and planning.
6. Conduct member visits and rounding as directed by the APC.
7. Update the APC on member status for evaluation and ICP modifications.
8. Communicate with members, families, and community caregivers during each visit.
9. Assist with clinical and quality responsibilities, addressing gaps in care.
10. Attend meetings on behalf of the APC and relay updates to the ICP with the ICT.
11. Provide clinical education programs for facility staff.
12. Relay information on behalf of the APC to members and caregivers.
13. Collaborate with health plans and clients.
14. Assist with Advance Care Planning (ACP).
15. Collect clinical data, including vital signs and other relevant health metrics, ensuring accurate entry into the Electronic Medical Record (EMR) for review by the APC.
16. Facilitate care coordination activities to meet member needs, including appointments and therapy requirements.
17. Stay informed about Model of Care (MOC) updates and maintain understanding of clinical resources.
18. Uphold confidentiality of health information in accordance with state and federal regulations, including HIPAA.

Qualifications:
Required:
- Current unrestricted Registered Nurse license in Washington.
- Minimum of 3 years of RN experience.
- At least 1 year of case management experience.
- Proficiency in using electronic medical records.
- Basic Microsoft Office skills and ability to navigate a Windows environment.
- Excellent communication and customer service skills.
- Valid driver's license with the ability to travel locally 75% of the time.

Preferred:
- Bachelor of Science in Nursing (BSN).
- Experience in post-acute care settings, such as long-term care or hospice.
- Familiarity with the geriatric population and long-term care business environment.

Compensation:
The salary range for this position is $58,300 to $114,300 annually, based on various factors including local labor markets, education, and experience. UnitedHealth Group adheres to all applicable minimum wage laws.
