RN Care Coordinator Skagit and Whatcom Counties

We are seeking a skilled Registered Nurse to join our team as a RN Care Coordinator Skagit and Whatcom Counties. This role will be responsible for coordinating care services for patients in the Skagit and Whatcom counties.
Key Responsibilities- Establish and maintain effective communication with patients, families, and healthcare providers to ensure seamless care coordination.
- Review patient status and develop individualized care plans to achieve progress toward goals.
- Perform administrative tasks, such as data collection and entry, to support care coordination efforts.
- Collaborate with healthcare teams to ensure timely and effective care delivery.
- Provide education and support to patients and families to promote health and wellness.
- Stay up-to-date with clinical resources and best practices to ensure high-quality care.
- Maintain confidentiality and handle sensitive patient information with discretion.
- Current unrestricted Registered Nurse license in Washington.
- 3+ years of RN experience, with 1+ year of case management experience.
- Experience with electronic medical records and Microsoft Office.
- Excellent communication and customer service skills.
- Valid driver's license and ability to travel locally 75% of the time.
- BSN degree.
- Experience working in post-acute care, such as long-term care or hospice, and/or case management experience as an RN.
- Experience working with the geriatric population.
- Familiarity with the long-term care business environment.
