  • Conduct assessments of member status and facilitate progress towards healthcare goals.
  • Perform administrative duties to support the Primary Care Provider (PCP) and Integrated Care Team.
  • Participate in program evaluation and planning to enhance service effectiveness.
  • Conduct member visits, both in-person and telephonically, to provide ongoing support.
  • Assist with clinical responsibilities, including addressing gaps in care.
  • Attend and represent the Integrated Care Team in meetings, providing updates on member care plans.
  • Educate facility staff on clinical best practices and care coordination.
  • Ensure compliance with health information confidentiality regulations.
Qualifications:

Required:
- Current, unrestricted Registered Nurse license in Oregon.
- Minimum of 3 years of nursing experience, with at least 1 year in case management.
- Proficiency in electronic medical records and basic Microsoft Office applications.
- Strong communication and customer service skills.
- Valid driver's license with the ability to travel locally.

Preferred:
- Bachelor of Science in Nursing (BSN).
- Experience in post-acute care settings, such as long-term care or hospice.
- Familiarity with the geriatric population and long-term care environments.
