Key Responsibilities- Establish and Maintain Relationships: Develop and maintain strong relationships with patients, families, caregivers, and healthcare providers to ensure comprehensive care coordination.
- Coordinate Care Services: Collaborate with interdisciplinary teams to coordinate care services, including hospitalizations, surgeries, and other medical interventions.
- Assess and Address Patient Needs: Conduct thorough assessments of patient needs and develop individualized care plans to address gaps in care and promote optimal health outcomes.
- Communicate Effectively: Communicate effectively with patients, families, caregivers, and healthcare providers to ensure timely and accurate information exchange.
- Monitor and Evaluate Care: Monitor patient progress and evaluate the effectiveness of care plans, making adjustments as needed to ensure optimal health outcomes.
- Current Unrestricted RN License: Possess a current, unrestricted Registered Nurse license in Washington State.
- 3+ Years of RN Experience: Have a minimum of 3 years of experience as a Registered Nurse, with a strong background in case management and care coordination.
- Electronic Medical Records Experience: Have experience using electronic medical records systems.
- Excellent Communication Skills: Possess excellent communication and customer service skills, with the ability to effectively interact with patients, families, caregivers, and healthcare providers.
- BSN Degree: Possess a Bachelor of Science in Nursing degree.
- Post-Acute Care Experience: Have experience working in post-acute care settings, such as long-term care or hospice.
- Geriatric Experience: Have experience working with the geriatric population.
