Job Summary

PATH is seeking a highly motivated and compassionate Outreach Case Manager to join our team at the Metro Line location in Los Angeles, CA. As an Outreach Case Manager, you will be responsible for providing street outreach and engagement services to connect clients with long-term housing and supportive services.

Key Responsibilities
  • Conduct daily street outreach to build relationships with clients and assess their needs
  • Develop Individualized Service Plans for clients to ensure their needs are met
  • Connect clients to supportive services and resources, including housing navigation and applications
  • Collaborate with community providers for seamless referrals and service coordination
Requirements
  • Two years of experience in outreach case management or a related field
  • Valid driver's license, reliable transportation, and proof of insurance
  • Ability to obtain CPR/First Aid training
