Position Summary:

The primary goal of our services is to assist individuals in Los Angeles County who require support in accessing housing, mental health services, substance abuse treatment, and community resources. The Intensive Case Manager plays a crucial role in establishing a supportive network to help clients avoid reincarceration and achieve long-term housing stability.

Key Responsibilities:

  • Conduct interviews and assessments to determine participant needs, collaborating with referrals from the Office of Diversion and Reentry.
  • Perform wellness checks at clients' residences and provide crisis intervention as necessary.
  • Develop personalized treatment plans to address the unique service needs and goals of each participant.
  • Assist clients in navigating public benefits and connecting them to necessary resources.
  • Coordinate access to mental health, substance abuse, and housing services.
  • Collaborate with team members to facilitate placement and case management post-release.
  • Engage with housing staff, probation officers, psychiatrists, public defenders, and primary care providers to ensure comprehensive support for clients.
  • Participate in inter-agency meetings, coalitions, and advocacy groups to enhance community cooperation and address participant needs.
  • Maintain thorough and accurate case records for all participants receiving services.
  • Provide timely documentation and case notes in accordance with funding guidelines.
  • Utilize both company and personal vehicles for client transportation and visits to various locations.

Qualifications:

  • A Bachelor’s degree in behavioral health or social science, along with one year of relevant experience in outreach or substance abuse counseling, OR
  • A high school diploma with three years of cumulative experience in outreach, substance abuse counseling, reentry services, and case management in diverse populations.
  • Proficiency in administering assessments and providing counseling within a culturally diverse context.
  • Strong organizational skills and attention to detail.
  • Excellent written and verbal communication abilities.
  • In-depth knowledge of non-profit operations.
  • Proficiency in Microsoft Office Suite or similar applications.
  • Possession of valid driver's license, vehicle, and car insurance is required.
