We are seeking a highly motivated and compassionate Community Outreach Specialist to join our team at Healthcare Staffing Professionals, Inc. As a key member of our Clinical Field Team, you will play a vital role in providing support to our Specialized Investigation Team activities and delivering specialized sexual and reproductive health-specific outreach and care to high-risk clients in non-clinical settings.
Key Responsibilities- Outreach and Engagement: Participate in outreach activities to reach highly vulnerable clients in non-clinical settings at risk of or experiencing syphilis, and conduct outreach and engagement with the community and vulnerable persons to inform about sexual and reproductive health care services offered.
- Field Support: Assist and support clinical staff with field testing and treatment, and assist with client tracking and patient reminders.
- Client Intake and Flow: Perform client intake and assist with patient flow during outreach, and contribute to case conference reviews and shares findings and lessons learned to improve programmatic response.
- Transportation and Medication Dispensing: Transport DHSP patients by county vehicle for sexual health specific appointments as needed and provide field medication dispensing for STD treatment.
- Resource and Advocacy: Serve as a resource and advocate for client access to departmental and community resources, and attend staff and team meetings and complete all required trainings to be compliant with departmental and divisional policies and protocols.
- A valid California Class C Driver License.
- Ability to operate a computer to obtain lab results, log clinic visits, and input/retrieve data.
- Locally based and open to working with street/homeless populations and out in the field in outreach locations as needed.
- Experience working with target populations to provide cultural and linguistically appropriate health education.
- Spanish speaking skills desirable, but not required.
- Excellent verbal, written communication and listening skills.
- Knowledge of the community and familiarity with Los Angeles street populations.
- Ability to drive the equipped van as needed.
- Ability to remain calm and be resourceful in emergency situations and maintain a professional demeanor at all times.
