PATH is seeking a highly motivated and compassionate Homeless Services Case Manager to join our Outreach Team in West Los Angeles. Key Responsibilities- Conduct Intakes and Needs Assessments: Meet with clients to understand their unique needs and develop personalized service plans to address housing goals, income, and other essential areas.
- Provide Referrals and Support: Connect clients with local resources, services, and organizations to help them achieve their goals.
- Evaluate Progress and Adjust Plans: Regularly review client progress, identify areas for improvement, and adjust service plans as needed.
- Maintain Accurate Records: Ensure timely and accurate documentation of client interactions, progress, and outcomes.
- Collaborate with Team Members: Work closely with colleagues to share knowledge, best practices, and resources to enhance client outcomes.
- Education and Experience: A minimum of 2 years of experience in healthcare, case management, homeless services, or a related field.
- Driving Requirements: A valid driver's license, reliable transportation, and proof of insurance are mandatory for this role.
- Background Checks: Candidates must undergo a background check and provide employment eligibility verification.
