Case Manager for Homeless Families

We are seeking a compassionate and experienced Case Manager to join our team. The ideal candidate will have a strong commitment to serving vulnerable populations and a proven track record of success in case management within the social services field.

Responsibilities
  • Provide comprehensive case management services to homeless families, including needs assessments, goal setting, resource referrals, and advocacy.
  • Develop and implement individualized service plans tailored to the unique needs of each family.
  • Coordinate with various community partners to ensure access to essential resources such as housing, food, healthcare, and employment services.
  • Maintain accurate and up-to-date client records, ensuring compliance with all relevant regulations and policies.
  • Conduct outreach and engagement activities to connect with homeless families in need of support.
  • Collaborate with DCFS and other agencies to facilitate the placement of homeless families into permanent housing solutions.
Qualifications

The successful candidate will possess:

  • A minimum of five years of experience working in the social service field, with a focus on case management and homeless program experience preferred.
  • Working knowledge of Microsoft Word, Excel, and other database programs. Familiarity with resource development, case management documentation, and HMIS is beneficial.
  • Bilingual proficiency in Spanish (read, write, and speak) to effectively communicate with diverse client populations.
  • Excellent communication skills, both written and oral, enabling clear and concise interactions with clients, colleagues, and community partners.
  • A deep understanding of the challenges faced by homeless families and a commitment to providing compassionate and effective support.
Additional Requirements
  • TB test required (within 3 months prior to or 7 days after hire, renewed annually thereafter).
  • CPR and First Aid Certification required within 30 days of employment.
  • Valid Driver's License and auto insurance are required. Reliable transportation is essential for conducting outreach and site visits.
  • Fully COVID-19 vaccinated (2 weeks from last vaccination dose to be considered fully vaccinated).
  • Negative COVID-19 PCR test prior to the first day of employment. Rapid antigen tests are not acceptable. The test date must be within 4 days (96 hours) prior to your start date.
