Job Overview

The Case Manager plays a vital role within a collaborative team of social service professionals, ensuring the provision of exceptional care while adhering to established guidelines and the mission of St. Joseph Center.

Key Responsibilities

The primary responsibilities of the Case Manager include:

  • Conducting outreach and providing navigation and support services to individuals facing homelessness.
  • Establishing and nurturing strong connections with community stakeholders, law enforcement, and other service providers.
  • Facilitating client access to suitable housing programs and assisting with necessary enrollment documentation.
Essential Duties

The Case Manager is expected to:

  • Deliver comprehensive case management tailored to individual client needs.
  • Build trust and rapport with clients experiencing homelessness, utilizing assessment tools to evaluate their situations.
  • Connect clients with essential health services and assist them in obtaining necessary identification and benefits.
  • Maintain organized client files and accurately input data into the Homeless Management Information System (HMIS).
  • Provide transportation assistance as needed, including bus tokens and taxi vouchers.
  • Engage in community meetings and case conferencing to enhance service delivery.
  • Collaborate with other outreach teams for joint initiatives.
Qualifications

Knowledge, Skills & Abilities:

  • Self-motivated with strong communication skills to engage with diverse populations.
  • In-depth understanding of homeless services and best practices in social work.
  • Bilingual abilities are advantageous.
  • Flexibility to work varied hours, including early mornings and evenings.

Experience:

  • A minimum of two years in providing services to vulnerable populations.

Education:

  • A Bachelor’s degree in Psychology, Social Work, Human Services, or a related field, or equivalent experience.

Technical Skills:

  • Proficient in Microsoft Office Suite and internet navigation.

Certificates and Licenses:

  • Valid California Class C Driver License and appropriate automobile insurance.
Work Environment

The work environment is typical of a social services setting, with reasonable accommodations available for individuals with disabilities.

Physical Demands

Employees may need to perform tasks that require standing, walking, and lifting up to 20 lbs as part of their job responsibilities.
