ABOUT OUR ORGANIZATION
Since our inception, we have developed innovative and effective strategies to support individuals facing homelessness. We provide a wide range of services across the state, connecting clients to essential resources including homelessness prevention, street outreach, employment assistance, individualized case management, and both interim and permanent housing solutions.
ABOUT THE OUTREACH TEAM
Our Street Outreach programs employ an integrated approach to engage individuals experiencing homelessness. The primary aim is to swiftly assess and connect these individuals with the necessary supportive services and resources to enhance their well-being, facilitate their transition off the streets, and secure long-term housing.
ROLE OF THE COMMUNITY OUTREACH COORDINATOR
As a vital member of the Outreach Team, the Community Outreach Coordinator's main responsibility is to assist clients in overcoming homelessness through a comprehensive support strategy. This role encompasses all stages of homelessness, including outreach, interim housing, transitional housing, and permanent housing.
Key Responsibilities:
- Conduct thorough intake assessments and collaborate with clients to create Individualized Service Plans (ISP) that focus on housing objectives, income enhancement, and other personal goals such as health, mental wellness, financial stability, and social support.
- Provide referrals and support to resources that aid clients in achieving their ISP objectives.
- Complete specific program assessments as required.
- Evaluate client strengths and challenges in relation to their short-term and long-term goals, conducting follow-up reviews as necessary.
- Maintain accurate documentation in compliance with program standards and organizational policies, ensuring timely updates to client files.
- Uphold client confidentiality in accordance with HIPAA and relevant guidelines.
- Establish and nurture professional relationships with clients and community partners to enhance engagement and support for housing goals.
QUALIFICATIONS WE SEEK:

PREFERRED EXPERIENCE:
- Two years of experience in healthcare, case management, or working with vulnerable populations is preferred.
MINIMUM REQUIREMENTS:
- All levels of education and experience are welcomed.
MANDATORY CRITERIA:
All candidates must meet the following requirements:
- Verification of employment eligibility.
- Willingness to obtain CPR/First Aid certification.
- Successful completion of background checks and drug tests as a condition of employment.
DRIVING REQUIREMENTS:
Driving is a critical component of this role. Candidates must possess:
- Reliable transportation.
- A valid driver's license.
- Proof of insurance and ownership for personal vehicles used for work-related duties.
