Overview

Under the guidance of the Program Manager and Program Supervisor, the Nurse Outreach will be an integral part of a multidisciplinary team, delivering professional clinical services aimed at outreach and treatment for individuals facing physical, mental, and developmental challenges. This role involves assisting clients and their support systems in recognizing and addressing issues stemming from health and emotional disturbances.

The Nurse Outreach will provide expert consultation and supervision to enhance skills in creating comprehensive bio-psycho-social assessments and developing effective health treatment plans aligned with multidisciplinary evaluations. Adherence to ethical and legal standards for documentation and reporting is essential.

The incumbent will apply evidence-based practices in client assessments, treatment plan implementation, and progress monitoring for various health conditions. Effective coordination of daily responsibilities in the field is crucial to ensure client service, documentation completion, meeting attendance, and training participation. Collaboration with the multidisciplinary team is key to the outreach efforts in the designated service area.

Key Responsibilities

  • Conduct thorough health assessments to gather data for comprehensive evaluations;
  • Collaborate effectively within a Multi-Disciplinary Team (MDT) to enhance service delivery;
  • Provide healthcare services to homeless individuals in various settings;
  • Develop client treatment plans based on health assessments;
  • Support clients in managing physical, mental, and substance-related challenges;
  • Document all activities in accordance with established guidelines;
  • Assist in identifying necessary case management and rehabilitation services;
  • Deliver nursing care in outreach locations, including shelters and community settings;
  • Assess immediate medical needs and facilitate access to urgent care when necessary;
  • Coordinate with team members to verify client healthcare coverage and assist with linkage to medical homes;
  • Engage with various healthcare providers to ensure comprehensive client care;
  • Provide ongoing nursing support for clients in their housing environments;
  • Conduct health screenings, basic assessments, and health education as assigned;
  • Maintain professional boundaries and adhere to ethical standards;
  • Participate in required meetings, training, and collaborative efforts;
  • Document client services in compliance with HIPAA and other regulations;
  • Assist with outreach and education regarding available programs and services.
Qualifications

  • Completion of accredited nursing training with clinical experience;
  • Active Registered Nurse license in good standing;
  • Ability to work with diverse populations, including homeless individuals;
  • Effective communication skills, both written and verbal;
  • Proficient in Microsoft Office applications;
  • Experience with co-occurring issues in homeless populations;
  • Ability to handle unpredictable situations and maintain composure under pressure;
  • Knowledge of evidence-based practices applicable in field settings.
Working Conditions

This position requires working in various environments, including areas with high rates of homelessness and potential hazards. The incumbent must be prepared for extensive interaction with individuals facing significant challenges, including health and safety risks. Local travel is required, and the ability to work effectively in diverse and sometimes challenging conditions is essential.
