Job Summary:

As an Outreach Case Manager, you will be responsible for providing street outreach and engagement services to connect clients with long-term housing and supportive services. This role involves conducting assessments, developing service plans, and collaborating with community partners to ensure clients' needs are met.

  • Conduct daily street outreach to build relationships with clients
