Job Summary
The People Concern is seeking a skilled Case Manager to join our team. As a Case Manager, you will play a critical role in supporting clients who are experiencing homelessness. Your primary responsibility will be to provide intensive case management services to a caseload of approximately 20 high-acuity clients.
Key Responsibilities
- Develop and maintain relationships with clients, focusing on facilitating independence and maintenance of improved physical and mental health.
- Collaborate with clients to assess and address barriers to housing placement, and work with them to implement interventions to support housing retention.
- Coordinate multiple services to meet individualized client needs, including outreach, interim housing, mental and medical health care, substance abuse services, and domestic violence services.
- Complete comprehensive biopsychosocial assessments and functional needs assessments to guide individualized treatment planning.
- Write and maintain accurate, complete, and up-to-date progress notes and data in electronic databases.
- Maintain confidentiality of client information per HIPAA and internal agency guidelines.
- Assist clients in applying for and obtaining all benefits to which they are entitled.
- Coordinate appointments, transportation, and follow-up services for clients to improve access to primary health care, mental health care, substance use services, and other local resources.
- Teach and model life skills, such as budgeting, housekeeping, meal preparation, appointment scheduling, health maintenance, and socialization within communities.
- Communicate effectively with property managers and other third-party partners.
- Work cooperatively and cohesively with multidisciplinary teams, including participation in weekly staff meetings, direct individual supervision, and trainings.
Requirements
- Bachelor's degree preferred; or a minimum two years' experience working with homeless individuals, preferably those living with mental illness and/or substance addictions, or an equivalent combination of education and experience.
- Valid Driver's license, reliable car, auto insurance, and an acceptable driving record.
- Willing and able to transport clients in your personal vehicle.
- Demonstrated knowledge of case management, preferred.
- Able to work in a high-tolerance Harm Reduction model with clients experiencing multiple barriers.
Work Environment
The Case Manager will work in a combination of field and office environments, regularly required to sit, stand, bend, and occasionally lift or carry up to 35 pounds. The position will necessitate working in busy and loud environments, and may require exposure to elements like cold, heat, dust, noise, and odor.
