Role Summary
The Case Manager plays a crucial role within a collaborative team of social service professionals, ensuring the provision of high-quality care in alignment with established regulations and the mission of St. Joseph Center. This position is integral to the Office of Diversion and Reentry Intensive Case Management Services (ODR ICMS), which supports individuals identified by the LA County Courts for participation in our program.

Key Responsibilities

  • Establish and nurture relationships with community stakeholders, law enforcement, and other service providers.
  • Assist clients in accessing interim housing solutions and guide them through the enrollment process for permanent housing programs.
  • Foster collaborative partnerships with other service providers involved in client care.

Essential Duties

  • Respond promptly to referral assignments and complete necessary tasks prior to client release.
  • Deliver case management and supportive services tailored to individual client needs.
  • Build trust with clients experiencing homelessness and conduct assessments using the Vulnerability Index (VI) SPDAT.
  • Connect clients with physical and mental health services, assist in obtaining identification, and enroll them in mainstream benefits.
  • Maintain organized client files and accurately enter data into the Homeless Management Information System (HMIS).
  • Provide transportation assistance as needed, including bus tokens and taxi vouchers.
  • Participate in case conferencing and community meetings.
  • Coordinate outreach efforts with other teams.
  • Ensure regular meetings with clients in various settings, including jails and homeless encampments.

Qualifications

  • Knowledge, Skills & Abilities: Must be a self-starter with strong communication skills to engage with diverse populations. Excellent organizational skills are essential, along with the ability to thrive in a fast-paced environment. Knowledge of homeless services and resources is crucial, along with experience in Harm Reduction and Housing First approaches.
  • Experience: A minimum of two years working with vulnerable populations is required.
  • Education: A Bachelor’s degree in Psychology, Social Work, Human Services, or a related field is necessary.
  • Computer Skills: Proficiency in MS Office Suite and experience with electronic health records is required.
  • Certificates, Licenses and Registrations: A valid California Class C Driver License and automobile liability insurance are mandatory.

The work environment is typically moderate in noise level, and reasonable accommodations may be made for individuals with disabilities.

Physical Demands
Employees may need to stand, sit, walk, and perform various physical tasks, including lifting up to 20 lbs.

Additional Information
