Job Overview

Position Summary: The Veteran Outreach Specialist plays a crucial role in enhancing the outreach efforts aimed at assisting veterans in need. This position involves collaborating with various stakeholders to ensure effective service delivery and support for vulnerable populations.

Key Responsibilities:

  • Lead initiatives focused on veteran outreach within designated service areas, including organizing case conferencing sessions with veteran service providers.
  • Collaborate closely with the lead agencies in the respective service areas and the Los Angeles Homeless Service Authority (LAHSA) to maintain and manage the Veteran's By-Name List.
  • Work alongside housing navigation teams to assist individuals experiencing homelessness in securing permanent housing solutions.
  • Conduct assessments using the Vulnerability Index-Service Prioritization Decision Assistance Tool (VI-SPDAT) during outreach activities.
  • Foster and maintain positive relationships with partner organizations, including the Veterans Administration and various community stakeholders.
  • Participate in staff training sessions and departmental meetings as required, along with other assigned responsibilities.

Essential Qualifications:

  • A minimum of two years of experience in a related role involving coordinated entry processes, assessments, and case conferencing.
  • Experience working with individuals facing mental health challenges and chronic homelessness.
  • Proven ability to coordinate with multiple agencies and service providers to facilitate assessments for those experiencing homelessness.
  • Familiarity with Service Prioritization Decision Assistance Tool (SPDAT) assessments and triage methodologies.
  • Strong organizational skills, initiative, and a commitment to follow-through.
  • Detail-oriented with the ability to prioritize tasks, meet deadlines, and maintain professionalism in conduct and appearance.
  • Proficient in office applications such as Word, Excel, Outlook, HMIS, and general internet usage.

Educational Background & Additional Requirements:

  • A Bachelor's degree is preferred; relevant work experience may be considered as a substitute.
  • Possession of a valid Driver's License, reliable transportation, current vehicle registration, proof of insurance, and eligibility for insurance coverage by New Directions.
  • Ability to successfully complete a background check, drug screening, and annual tuberculosis test.
