Compensation: $36.00-$45.47 (Dependent on Experience).
Location: Central Oregon
Work Schedule: Options for both part-time and full-time positions.
Travel Requirements: Local and regional travel using personal vehicles during work hours, with mileage reimbursement available.

Key Responsibilities:
  • Perform comprehensive initial evaluations of patients and their families to assess home health requirements.
  • Develop, monitor, and modify personalized care plans with measurable objectives based on therapy evaluations, patient capabilities, and medical conditions.
  • Implement suitable preventative and rehabilitative treatments.
  • Provide education to patients, families, and caregivers regarding physical therapy protocols and home exercise regimens.
  • Engage actively in the Fall Intervention Program.
  • Address discharge-planning considerations within care plans.
  • Revise the Home Health Aide Plan of care as necessary.
  • Oversee and assess home health aide services biweekly.
  • Participate in staff meetings, case discussions, and interdisciplinary team sessions as required.
Qualifications:
  • A degree from an accredited baccalaureate or master's program in physical therapy.
  • Valid licensure to practice as a physical therapist in the state.
  • A minimum of two years of relevant experience as a physical therapist, with a preference for community/home health experience.
  • Ability to successfully complete a background check, drug screening, and TB test.
  • Current CPR Certification.
  • Valid automobile insurance, a current driver's license, and dependable transportation.
  • Excellent communication and interpersonal abilities.
  • Capacity to work both independently and collaboratively within a multidisciplinary team.
