Responsibilities include:

  • Handle incoming calls from potential participants and follow up on referrals
  • Ensure smooth PACE eligibility and enrollment processes
  • Schedule home visits and assessments
  • Assist with various outreach and enrollment projects

Requirements:

  • Associate's degree, bachelor's degree preferred
  • 2 years of relevant experience
  • Strong customer service skills
  • Proficiency in Salesforce, MS Office Suite, or Athena is a plus
  • Bilingualism is advantageous
