Position Overview:

The Community Health Worker (CHW) I at Bay Area Community Health plays a crucial role in supporting and assisting patients across various locations and community initiatives. This position is designed for individuals who will serve in diverse capacities based on their specific training and expertise. The CHW will act as a trusted resource within the community, facilitating connections between primary care, specialty services, and available resources for patients. Responsibilities may encompass outreach, patient engagement, care coordination, case management, health education, and linking patients to essential services both within the organization and the broader community, including food, housing, financial assistance, and transportation.

Key Responsibilities:

The following duties represent typical tasks performed in this role. They are not exhaustive and may evolve over time. Under the guidance of the supervisor/manager, the Community Health Worker will undertake a variety of tasks, including:
  • Achieving CHW Certification after completing 2,000 hours of service.
  • Scheduling appointments in the Electronic Health Record system, in line with providers' scopes of practice.
  • Utilizing Microsoft Office for effective communication and reporting.
  • Conducting patient screening surveys, scoring questionnaires, and notifying the care team.
  • Documenting patient interactions in electronic health records.
  • Participating in health fairs and community outreach events to promote services.
  • Maintaining logs as directed by supervisors.
  • Following up on missed appointments.
  • Coordinating in-person interpreters and transportation through patient health plans as necessary.
  • Demonstrating reliability and independence in all tasks.
  • Providing outreach and enrollment services for public benefit programs.
  • Connecting patients with internal resources and services to address their needs.
  • Acting as a liaison between the community and health/social services.
  • Collaborating effectively with clinic staff, community members, and care teams.
  • Meeting departmental standards for documentation and review.
  • Adhering to the policies and procedures of assigned clinics.
  • Building and maintaining positive relationships on behalf of the organization.
  • Fostering a supportive work environment and promoting teamwork.
  • Upholding a positive image of the organization both internally and externally.
  • Working flexible hours, including evenings and weekends as required.
  • Providing services at various locations, including clinics, mobile units, schools, and community centers.
Additional Responsibilities:
  • Attending workshops, training sessions, and meetings as needed.
  • Performing other duties as assigned by the supervisor.
Qualifications:

Required Education, Experience, and Training:
  • High School Diploma or equivalent.
  • Two (2) years of experience in a community health service role preferred.
  • Bilingual candidates are preferred.

Work Conditions:
  • Travel to various sites may be required.

  • Flexible hours, including evenings and weekends, may be required.
Physical Requirements:
