Position Overview

At WelbeHealth, we are redefining senior care by offering a comprehensive healthcare solution tailored to our most at-risk senior population, acting as both a care provider and care planner for the participants we support.

Under the supervision of the Marketing, Outreach, Enrollment, and Eligibility (MOEE) Director, the Outreach and Enrollment (O&E) Coordinator is responsible for initiating, managing, and organizing the PACE eligibility and enrollment process. This role emphasizes customer service aligned with our mission, vision, and values.

Key Responsibilities:

  • Respond to inquiries from potential participants and conduct follow-up communications to referrals, ensuring exceptional customer service while clarifying the WelbeHealth enrollment process and converting inquiries into home visits.
  • Direct potential enrollments to the Benefits Coordinator when financial concerns arise, including share of costs (SOC) or Medi-Cal applications for financial assessment.
  • Exhibit comprehensive knowledge of all relevant aspects of the MOEE Playbook.
  • Coordinate initial home visits, LOC visits, MSW/PCP assessments, Enrollment Conferences, and transportation logistics for all appointments.
  • Support the MOEE Director and team in facilitating MOEE meetings, utilizing available A/V tools to document tasks, follow-ups, notes, and assignments within Salesforce.
  • Assist with public relations, marketing, outreach, and enrollment initiatives as required.

Qualifications:

  • Associate's degree in a relevant discipline; bachelor's degree preferred.
  • A minimum of two (2) years of relevant experience is preferred.
  • Strong commitment to customer service.
  • Ability to work autonomously with minimal oversight.
  • Reliable transportation is essential.
  • Experience with Salesforce, MS Office Suite, and/or Athena is preferred.
  • Bilingual skills are advantageous.

The salary range for this position is $30.44 hourly plus bonuses and equity. WelbeHealth offers a competitive total rewards package, including 401k matching, healthcare coverage, and a wide array of other benefits. Actual compensation will be adjusted based on experience and qualifications.
