Under the supervision of the Marketing, Outreach, Enrollment, and Eligibility (MOEE) Director, the Enrollment and Outreach Specialist is responsible for initiating, coordinating, and managing the PACE eligibility and enrollment process.

This role emphasizes customer service aligned with our mission, vision, and values.

Key Responsibilities:
Respond to inquiries from potential participants and conduct follow-up communications to referrals, delivering exceptional customer service, clarifying the WelbeHealth enrollment procedure, and converting inquiries into home visits.
Direct potential enrollments to the Benefits Coordinator when financial issues arise, including share of costs (SOC) or Medi-Cal applications necessary for financial assessment.
Exhibit comprehensive knowledge of all relevant aspects of the MOEE Playbook.
Organize initial home visits, LOC visits, MSW/PCP assessments, Enrollment Conferences, and transportation arrangements for all appointments.
Support the MOEE Director and team in facilitating MOEE meetings, utilizing available audio/visual tools to document tasks, follow-ups, notes, and assignments of the MOEE team in Salesforce.
Assist with public relations, marketing, outreach, and enrollment initiatives as required.

Qualifications:
Associate's degree in a relevant discipline; bachelor's degree preferred.
Minimum of two (2) years of relevant experience preferred.
Strong commitment to customer service.
Ability to work autonomously with minimal oversight.
Reliable transportation is necessary.
Experience with Salesforce, MS Office Suite, and/or Athena is preferred.
Bilingual candidates are preferred.

Actual compensation will be adjusted based on experience and qualifications.
Compensation range: $25.36—$30.44 USD
