Work schedule: Monday to Friday, 8:30 AM to 4:30 PM, with weekend and holiday rotations.

Summary:
The Patient Intake Coordinator plays a vital role in managing the intake, scheduling, and care delivery processes for patients. This position involves conducting interviews with patients to gather essential intake information and making referrals to service providers based on individual patient needs and assessments. The Coordinator operates under the general supervision of the Admission Team Manager.

Key Responsibilities:
  • Handles intake and informational calls, directing department inquiries to the appropriate personnel and assisting with outreach to non-admitted patients to facilitate admissions.
  • Assists in the daily coordination of clinical functions within the team, facilitating and scheduling assessments.
  • Collects complete demographic and insurance information from relevant sources. Secures medical certification and prognosis for Hospice patients from attending physicians prior to assessment visits.
  • Retrieves pertinent medical information related to referrals before nursing assessments when feasible.
  • Alerts the Team Manager or Director regarding scheduling issues and documentation or insurance requirements.
  • Ensures nursing staff has up-to-date and accurate patient information attached to the patient chart.
  • Applies local coverage determination policies to identify the most suitable care system for prospects and ensure eligibility for Hospice services.
  • Understands and applies Medicare, Medicaid, and third-party eligibility requirements, informing referral sources as necessary.
  • Participates in the verification of insurance benefits and follows up on third-party insurance authorizations when needed. Acts as a resource for insurance information for all staff members.
  • Engages in special projects, statistical reporting, and analysis as required.
  • Demonstrates a proactive approach, exhibiting a can-do attitude, the ability to multitask, and the capacity to manage challenging situations effectively.
  • Performs necessary clerical tasks, including filing, mailing, record keeping, copying, and other assigned duties to maintain organized medical and statistical records and facilitate referral processing.
  • Enters all patient information, telephone calls, and prospect dispositions into the Electronic Medical Record (EMR) system.

Qualifications:
  • Education: High School Diploma or GED preferred.
  • Experience: Prior experience in a healthcare setting is preferred, with a background in Hospice and Palliative Care being advantageous.
